PROGRAM COORDINATOR FOR MEDICAL IHPS
This position reports to Director of Medical IHPS
DUTIES:
Direct service to small caseload.
Provides comprehensive social work supervision to unit members via support, education/training, and interpretation of agency policies and procedures, in order to enhance the provision of services to client families.
This will be "operationalized" as follows:
- Manages unit caseload size e.g. assignment, transfer, and closing of cases.
- Assists with ongoing case assessment and planning, including safety and risk assessment, goal setting and strategies for intervention.
- Participates in review of cases, including internal or external case conferences, MDT reviews, and risk management meetings.
- Provides direct supervision to unit members on a weekly basis, unless otherwise determined by current agency policy.
- Conducts unit meetings on at least a bi-weekly basis to disseminate information, promote group cohesion and address agency policies.
- Collaborates with network of professionals in order to facilitate inter-agency communication.
- Participates in training activities and community education and public relations events in order to disseminate information on the rights of children with special health care needs and other related topics to staff, other professionals and the general public.
- Serves on intra-agency committees in order to address agency and client needs.
- Assists in recruitment/selection process for new staff as necessary.
- Performs related work as required.
KNOWLEDGE AND SKILLS REQUIRED:
- Considerable knowledge of human behavior, child welfare and other social systems and institutions.
- Advanced knowledge of social and environmental issues and several intervention methods or treatment approaches.
- Thorough knowledge of community resources relevant to identified needs and ability to negotiate complex systems.
- Ability to intervene and demonstrate appropriate social work techniques with clients in highly complex situations.
- Ability to cultivate effective working relationships with professional colleagues.
- Ability to organize verbal and written presentations.
- Ability to teach and/or convey expertise to staff through supervision or other forums.
- Strong leadership skills, with the ability to be supportive and respond to multiple staff needs and engage, when necessary, in effective problem-solving in both crisis and chronic situations.
- Strong organizational and managerial skills.
- Ability to participate in a team approach to long-range planning within the agency, keeping in mind the holistic needs of the client community served.
- Bilingual ability preferred.
- Working Knowledge of PC/Windows.
EDUCATION AND EXPERIENCE REQUIRED:
- MSW
- License preferred
- 5 years post &mdash masters experience in child welfare
- Supervisory experience preferred
CASE MANAGER—MEDICAL IHPS
This position reports to the Coordinator of Medical IHPS
DUTIES:
Provides social work services to families that have been identified for in &mdash home services to enhance the safety of children in need of protection who also have special medical needs.
Provides services, primarily in family home, which include but are not limited to:
- assessing family functioning and family needs in order to determine safety and well-being of children and develop appropriate goals and interventions.
- assisting family and providing training in parenting and life skills, budgeting, family health care, child development, discipline, family relationships, and emotional needs of the family.
- assisting family in identifying and making responsible use of community resources to meet families needs.
- Serves as a liaison and advocate with community-based resources such as schools, hospitals, drug and alcohol and mental health facilities in order to maximize family functioning.
- Collaborates with legal representatives in order to protect the welfare of the child and provides testimony in court as necessary.
- Participates in multi-disciplinary team meetings and case conferences in order to ensure quality services to families.
- Serves on committees as needed in order to address agency and client needs.
- With supervisory recommendation/approval, participates in the on-call emergency service rotation.
- Maintains thorough records as required by funding sources and the Agency.
- Performs related work as required.

CASE AIDE—MEDICAL IHPS
GENERAL DESCRIPTION
This is a home visiting position responsible for delivering service and support to clients and their families that have been identified for medical In &mdash Home Protective Services.
This position reports to the Program Supervisor for Medical IHPS
DUTIES
- Support families who are high risk for child abuse and neglect.
- Provide intensive life skills and parent skills training.
- In conjunction with social workers and supervisor, assess risk/need factors, which will determine appropriate levels of care.
- Collaborates with other members of the support team as well as other agencies and service providers to create individualized service plans.
- Maintains thorough records and required by funding sources and SCAN
- Attends case reviews and other meetings.
- Performs related work as required.
KNOWLEDGE AND SKILL PREFERRED
- General knowledge of community resources relevant to identified needs.

NURSE
The Nurse provides in-home assessment, monitoring and case management services in conjunction with other team members and consultants to families involved with the Department of Human Services and / or the Philadelphia Department of Public Health.
DUTIES
- Develops implements and monitors care plans and provides case management services.
- Provides consultation and training to SCAN staff regarding health issues.
- Coordinates and conducts intake assessments at collaborating hospitals and in clients' homes.
- Collaborates in conjunction with other members of SCAN’s Multidisciplinary Team on assessment and service plan development for SCAN families.
- Attends and participates in relevant agency and inter-agency meetings.
- Provide on going home visits for support, education and monitoring to families where a health care, and safety risk has been identified.
- Educate health care providers about child abuse and neglect and special issues regarding families in the child welfare system.
- Performs other duties as required.

The Program Secretary reports to the Office Manager.
DUTIES

- Files client materials and other record information.
- Answers telephone and records and distributes messages.
- Operates mail, schedules, rooms, and other related functions in absence of the Office Manager.
