Director of Specialized Home Based Services
(Services to Children in their own Homes)
The Director of Specialized Home Based Services position combines a high level of professional health and/or human service knowledge and skills with some administrative functions, and reports to the Clinical Operations Director. Responsibilities include: 1) coordinating the delivery of specialized home based child welfare services; 2) assuring a high quality of clinical practice; and 3) ensuring compliance with the terms of contracts relating to the delivery of outreach services.
DUTIES:
- Supervises Outreach Social Work Unit
- Monitors the quality of work performed by outreach staff in compliance with accepted standards of clinical practice and governmental regulations. Works closely with the Continuous Quality Improvement Director
- Ensures the implementation (for home based child protective service staff) of all Agency's operational and personnel policies.
- Participates in the coordination of in-service training.
- Assesses the need for and contributes to the development of, additional and alternative services for client families including homemaking, group work, day care/day treatment and respite care.
- Collaborates with and ensures positive communication with all professional specialists within the Agency. Develops working relationships with various health and human service providers, governmental agencies and professional schools and associations, including social work, nursing and other disciplines involved in child abuse research, planning and program evaluation.
- May provide short term supervision for new home base projects.
- Provides monthly reports to the Clinical Director.
- Performs related work as required.
REQUIREMENTS:
MSW, MSN or equivalent degree from an accredited school License preferred.
At least seven years of clinical experience, including two years of "outreach" and two years of supervisory experience in health or human services.

Supervisor of Rapid Service Response Initiative
This position reports to the Clinical Operations Director
Provides comprehensive social work supervision to unit members via support, education/training, and interpretation of agency policies and procedures, in order to enhance the provision of services to families.
This will be "operationalized" as follows:
- Manages unit caseload size i.e assignment, transfer, and closing of cases.
- Assists with ongoing case assessment and planning, including risk assessment, goal setting and strategies for intervention.
- Provides direct supervision to unit members on a weekly basis, unless otherwise determined by current agency policy.
- Conducts unit meetings on at least a bi-weekly basis to disseminate information, promote group cohesion and address agency policies.
- Participates in training activities and community education and public relations events in order to disseminate information on child abuse and other related topics to staff, other professionals and the general public.
- Collaborates with network of professionals in order to facilitate inter-agency communication.

- Serves on intra-agency committees in order to address agency and client needs.
- Serve on agency's representative at all RSRI meetings, conferences, etc.

IMMUNIZATION PROJECT COORDINATOR
GENERAL DESCRIPTION:
The Project Coordinator is responsible for directing the immunization project as defined by the Philadelphia Department of Public Health (PDPH)
The Project Coordinator reports to the Director of Clinical Operations.
DUTIES:
- Supervises and provides technical assistance to Home Visiting staff on a regular basis.
- Develops and implements program policies and procedures based on PDPH policy guidelines.
- Monitors immunization plan.
- Organizes and leads community forums eg. Churches, including media contact.
- Conducts field work for back-up and monitoring of services
- Supervises data collection and management
- Establishes and maintains relationships with primary care providers
- Attends and participates in relevant agency and inter-agency meetings.
- Assists in recruitment/selection process for new team staff.
- Performs related work as required.

SUBSTANCE ABUSE SPECIALIST
Substance Abuse Specialist will work to reduce the risk of poor health outcomes for infants and children in families where the caregiver substance abuse is identified.
This position reports to the Project Coordinator.
DUTIES
- Collaborate in the initial assessment and service plan development for family's clients where caregiver substance abuse has been identified, including assisting in determining the level of risk to a child posed by a caregiver's substance abuse.
- Determine family functioning and needs in order to determine safety and well-being of children and develop appropriate goals and intervention.
- Develop relationships with drug and alcohol abuse treatment programs and facilities in order to assist family's staff in identifying and accessing appropriate treatment resources for substance abusing clients.
- Collaborates in conjunction with other members of SCAN's Multidisciplinary Team on assessment and service plan development for SCAN families
- Provide referrals and linkage for substance abuse treatment either through direct work with the family or in collaboration with the Home Visiting Team.
- Providing home based support in collaboration with the families in cases where substance abuse is present but caregiver is unwilling to accept referral for treatment.
- Provide consultation and training to family's home visiting teams regarding substance abuse issues, including understanding limitations of family home setting as an intervention strategy where untreated substance is present.
Provide consultation to family's staff regarding appropriate termination of voluntary services to families where untreated substance abuse continues to pose a threat to children's health. Assist family's staff in effecting referral of these families to DHS for child protective services.
Provide education and training for family's referral sources regarding substance abuse issues and the substance abuse treatment system.

- Serves as a liaison and advocate with community-based resources such as schools, hospitals, drug and alcohol and mental health facilities in order to maximize family functioning.
- Collaborates with legal representatives in order to protect the welfare of the child and provides testimony in court as necessary.
- Participates in multi-disciplinary team meetings and case conferences in order to ensure quality services to families.
- With supervisory recommendation/approval, contributes to training activities and community education and public relation events in order to disseminate information on child abuse and other related topics to staff, other professionals and the general public.

PROGRAM SECRETARY
This is a secretarial position involving typing, data entry, answering phones, copying, filing, and performing related clerical functions.
The Program Secretary reports to the Office Manager.
DUTIES
- Types reports and correspondence including client reports, letters, memos, minutes and other typing as directed.
- Input data into program’s database
- Files client materials and other record information.
- Answers telephone and records and distributes messages.
- Operates mail, schedules, rooms, and other related functions in absence of the Office Manager.
- Assists with bulk mailings.
- Performs related clerical functions, as necessary, including back-up to other SCAN office personnel.
